Hi Maxim,

I feel, there is one case also failing in demo also:

1. upload image file.
2. annotate that file and save.
3. annotate that file again which is annotated already in step 2.

If same thing happened here, can guide Vaibhav too.

I just have tried "vanilla" openmeetings and everything works as expected File 
hash is unique by design (and by implementation since it is UUID), I'm not sure 
why you need another unique identifier

you should use debugger to check what is going on in your modifier code :(
