if you need a cross-region cluster, yeah, things get way more complicated unless your regions are fairly close. I've run datacenters before within 20 or so ms of eachother (stable dedicated links like wavelengths, not public internet) with a stretched galera with success, mostly because of the opportunistic certification ... but I didn't explicitly try that with cloudstack.
