just for the record (and the mailing list archives :), I want to mention that you submitted this as issue 54492 (http://www.openoffice.org/issues/show_bug.cgi?id=54492). Problem here is that once you manually close the Form Filter toolbar, you don't have a chance to re-activate it :(. (ugly) Workaround described in the issue.
