I have attached my attempt at this.  It wont get to the list but hopefully it 
might get to Gary.  


It was a bit of a pain because small chunks needed to be done as columns but 
then a line or 2 would need to be done with no columns (or just 1 column 
depending on how you look at it).  Also depending on font the page-breaks come 
at inconvient opints so it only seemed to work easily for me if i switched View 
from "Print" to "web-layout"
