If so, your old user profile might be the cause (the "automagic" migration from 3.x to 4.0.x possibly didn't work properly). Failing that, 4.0.4.2 is pretty old, and 4.0.4 also contained a fair number of bugs which have since been fixed in more recent versions of LO.
