The TOC is built using the outline levels of the heading styles you assigned to
chapter headings and subsidiary headings. If the book title has been included, did
you use H1 for the title and author names? If you used a different paragraph style
for the Title and author names, you can check that those styles do not have outline
levels set (Edit style > Outline and list tab > set outline level = None).
The page numbers can be set on the page styles. It would be common practice to
define a different page style for some of the front matter. For example, you
could create a Preface style with Left and Right pages and set those to have
page numbers in Roman numerals and then the main page style could start with
page 1 in Arabic styles. With a selection of page styles, each can have headers
or footers selected or not, as you wish. I would typically have six or seven
different page styles in a book in addition to a mirrored default page style.
To eliminate the dotted lines in the TOC, right click in the toc and select
Edit Index. Select the Entries tab and select the Level for which you do not
want the dotted lines. Then in the Structure line for that level, click on the
'T' box and a dialog opens below allowing you to select 'Fill character' =
None. Then click OK.
