I did tried the most updated cvs. 
Yes, it can be compiled but returned error:
Group 'mysql-connection' may not contain field 'max-connections'
Removing the line of 'max-connections' just lead to another error like:
Group 'mysql-connection' may not contain field 'group'

Indeed it does not compile, because MySQL 4.1 headers define some 
functions which are also defined in Kannel gwlib/list.h. Alex fixed this 
a long time ago in CVS, right after the 1.4.0 release.

Give the code in CVS a try, it's quite stable and it will compile with 
MySQL 4.1 headers.
