Looking at the reply URL, it looks like you have set the PDF reply URL and not the portal URL. Can you check whether you have set the PDF reply URL? Normally you should set the portal base URL. All other URLs will be automatically use the portal base URL as the base of the URL. Only on special circumstances you need to manually set the PDF reply URL.
