The target (null) is probably a bad thing. The spec doesn’t say if an empty value and a null value are equivalent, but on the client side they definitely aren’t.
Try putting in a value for <TargetFileName> and see what happens. I.e. have you tried pointing your browser at the link in the <URL>http://xxx.xxx.xxx.xxx:7567/firmware-1.img</URL>? I had issues for the longest time getting the firmware upgrades working. Turns out I had forgotten to open the 7567 port on the firewall. Also, check the log on the device, it will probably tell you why it didn't perform the requested action. Depending on the CPE you have, you may be able to get more information if you connect to the serial console on the device. To do that, you will need to take the cover off the CPE. There will be a 4 pin header on the circuit board. If you have a logic analyzer, you can figure out the pinout really easily, otherwise it requires some trial and error. Then connect up a serial breakout cable<https://www.sparkfun.com/products/9717> to the device using jumper wire<https://www.sparkfun.com/products/9385> and fire up your terminal program with 115200,8N1. If you do get access to the console, you can then enable the soap messages by setting InternetGatewayDevice.X_BROADCOM_COM_AppCfg.Tr69cCfg.LoggingSOAP to true. All of the ZyXEL, SmartRG devices I've opened up have the serial header. The BEC's and Thomson's do not.
