you mean you get the call (from main SIP server), you send the INVITE to the client (he is behind a NAT), and you receive back from client a 401 reply ?

I can complete outbound calling and registration via OpenSIPS using rewritehostport – but I cannot receive calls to the UA.

From the sip logs it appears to be returning the call to the UA directly (public ip) and not via OpenSIPS or via private NAT ip.
