I don't have the files here to revert to OOo1.1.2, but using 1.1.4 it is easy
(if I understand what you're asking).
1. Open the template or the document you will save as a template.
2. Position the cursor where the field is to be placed.
3. Menu Insert|Fields|Other
4. Select the Functions Tab and Input Field type
5. Click on Insert which opens a box labelled Input Field
6. In the bottom window, enter your prompt. I usually enclose it in <arrow
brackets> so it stands out on the page.
7. Click OK then Close
8. Save as template
I like the fact that OOo prompts for completing the Input Fields when the
template is opened. Of course, you can always select the fields individually.
