YAY!
I was feeling your pain because I went thru much the same problem a week or so ago. Much swearing and muttering was emanating from my cubicle. I'm just adding this as a tip...
<tip author="Naomi Kramer" subject="Including Additional Headings in TOC (OOo 2.0)" category="Writer">To include in the TOC headings which aren't in the outline numbering -
1. Click on Insert -> Indexes and Tables -> Indexes and Tables
2. Select Table of Contents from the Type drop down list
3. Select the number of levels of headings to use (I tend to only use 4)
4. Tick the Additional Styles checkbox
5. Click on the ... button next to said checkbox
6. Select the headings you want in the TOC (any you've already using with Outline Numbering will go in automatically... and/or the default Heading1 to HeadingX styles) and click on the >> button to move them to the desired level.
7. Click on OK.
If you are editing an existing TOC and the changes won't save, delete the current TOC and re-generate it.</tip>

Bingo! deleting the index and recreating worked, and, the additional styles stuck this time. thanks Naomi.
