Or someone who is not subscribed to the list and therefore never saw any of
the to-the-list-only replies. To see whether a user is subscribed or not
simply filter for the field "Delivered-to:" and if it contains "moderator"
then you should really CC the person. Note that most "simple" questions are
from such posters.
I CC'd him with my response, so that's not the totally it, either.
