Howdy!  I've written a book that makes extensive use of hyperlinks. 
If people view the book in electronic format, as .html or as .sxw,
they can follow the hyperlinks direcly.  However, when submitting my
book to editors and publishers as a printed manuscript, obviously, the
reader can't simply click the link and follow it to the approprieate
chapter.

I know how to use a table of contents with openoffice.org, my question
is, is there a way to automatically append the appropriate page number
to each individual hyperlink?  That way, readers of the hard copy
won't have to consult the table of contents, they can just go to the
appropriate page number.
