It may be a case of a difference between en-US and en-UK, or two usages with 
equal value.   Or it may simply be an error on the form.  But I would 
certainly say "fill in", not "fill out".  There is nothing wrong with your 
English here.

(Have just checked in Chambers 20th Century Dictionary.  According to 
it, "fill in" means to complete a form, fill out means to increase in size.)
