o  In the table of contents, go to right-click | Edit Index/Table | Entries.
o  Click the "LS" button itself (may appear to be "IS").
o  Press the Delete key on the keyboard.
o  Click the "LF" button itself (may appear to be "IF").
o  Press the Delete key on the keyboard.
o  Press OK.

Alternatively, provided the Insert Index/Table | Index/Table | Type and title | "Protected against manual changes" box is *not* ticked, you can select hyperlinked index entries and use Format | Default Formatting (or right-click | Default Formatting, or Ctrl+M) to unlink them individually.
