Getting closer, but still much confusion in my alleged mind.
(Talking only of 2.4.1 here, because I think the first step is to fix
the problems in its Extension Manager. Then the migration to 3.0 might
work properly. The following is a log of what I have done that has
resulted in almost resolving the problem.)
OK, when I opened Tools > Extension Manager I saw two entries:

Interestingly, I renamed the
folder /usr/lib/openoffice/share/extension/install by adding .old to
its name. Made no difference. I restarted 2.4.1 and the above three
were still listed under OpenOffice.org Extensions.
I thought I might just uninstall all of them to see what would happen.
But in Extension Manager the only buttons that are available for the
OpenOffice.org Extensions folder are Export and Updates. The Add,
Remove, Enable, Disable and Options buttons are grayed out. For the My
Extensions folder the Add and Updates button are the only ones
available. I tried Add, but immediately got the same error message
that posted previously.
In ~/.penoffice.org2/user/uno_packages/cache/uno_packages I had the
following folders:

I opened the log.txt file in Gedit and it is thousands of pages of
error messages.
In the above list four folders were added by me this morning (1He3I6,
arRg3r_, bJjRTc_, and OWqG1Q_). That is because when I first tried to
open My Extensions in Extension Manager I got error messages that (for
example) there was no such file
as ///1He3I6/Readability_2004-03-03.zip. I had the file, but it was in
a higher folder. So I manually created the folder and copied the file
into it. I got four error messages like this and eliminated them all
one by one by manually adding the folder and putting the correct file
in it. But at the end I got the "(com.sun.star.uno.RuntimeException)"
error message, and I couldn't get past it.
I should add that all of these extensions were working fine in 2.4.1.
So then I renamed
~/.penoffice.org2/user/uno_packages/cache/uno_packages by
appending .old to it. On restarting OOo I found that it recreated the
folder, and with nothing in it. I went to Tools > Extensions Manager
and selected My Extensions. This time when I clicked on Add I got a
browser. Yay! I can just re-add the extensions.
Now what is happening in 3.0:
I renamed the ~.openoffice.org folder by appending .1218 to it. Then I
launched 3.0. Again it came up and asked the same questions, incuding
if I wanted to migrate my 2.4.1 settings. I left the box checked "yes"
and proceeded with the rest of the questions. When it launched
everything seemed to be working fine. I opened a Writer document and
went to Tools > Extensions Manager. This time it came up as it is
supposed to. I added the Zotero extension and closed the Extensions
Manager.
I had to restart 3.0 before the Zotero extension would appear, but it
came up fine. The toolbar appeared properly, and when I clicked on the
Insert Citation button it popped up a window asking for the style I
wanted to use. It was even still set by default to APA, as it had been
in 2.4.1.
But that's the end of the good news. When said OK to the APA style the
window changed to the browser window where I can select the work I want
to cite (from my libraries created in the Firefox Zotero plugin).
Unfortunately, the window would not respond further. I discovered that
it would not respond to the mouse, but it would respond to the
keyboard. So I tabbed to the Cancel button, but even that took several
tries before I finally succeeded the window. Afterwards I was able to
exit OOo 3.0 gracefully and without error messages.
I'm making a lot of progress, thanks to your help. I'm documenting this
saga here so that others will know that if there is anything at all
wonky in their extensions in 2.4.1 an upgrade to 3.0 will have problems.
Now on to figure out what is wrong with Zotero.
