What is puzzling is the icon association worked with Open Office version 2, but started this after I installed version 3.

Right click on one of the blank icons. Click 'Get Info'. Toward the bottom of the info window it says 'Open with'. Select the appropriate version of OO.o. Below that is 'Use this application to open all documents like this' - click 'Change All'.
