It's perhaps worth saying that having a case-sensitive dictionary can be useful - with proper names, for example. My OpenOffice English (UK) dictionary, for example, accepts "Jonathan" but rejects "jonathan" - which is helpful. (In typing this, I notice that my mail client's spelling checker does the same.)

But your point is different from this: that if you want a lower-case version included you are likely to want a initial-capitalised version of the same word accepted too. I'm guessing, but I imagine that default dictionaries include complex data indicating which words can appear in which inflected forms. And I'm also guessing that custom dictionaries could not do that unless the user were quizzed in detail about the particular properties of the word they were adding.
