In general OpenOffice.org is free to use in both commercial and
non-commercial situations and can be given out freely to your staff as
well.
Licenses are at http://www.openoffice.org/license.html
you may like to have your lawyers check that these are appropriate for
your use.
There are medical dictionaries available on the Extensions service, in
US and Australian flavours.
