I believe I did everything correctly but still no table of contents. I

headings but no TOC appeared when I hit OK. Please see if you can find

There is a link missing between the headers and outline numbering.
To solve it: Choose Tools|Outline Numbering in your document and you'll
see that for level 1 (2, 3) no style is selected. Choose Header 1 (etc,
or any other style that you use) there, and it works.
