You can use either, though as a rule of thumb, I would probably use a spreadsheet if a single person is entering the information and a database if more than one is using it. I haven't done it, but I can imagine somebody else doing this. as for online or shared, if you are going that route it would probably be better to have a full RDBMS like MySQL or PostgreSQL and using Either Base or Calc as the front end.

Do any listmembers make use of - or have created their own - database or

How about similar for health/medical tracking, with info like (but not limited
to) weight, physical dimensions, meds, symptoms (occurrence, intensity, etc.),
side-effects (occurrence, intensity, etc.), treatment progress for
major-illness recovery (heart, stroke, cancer, diabetes, other), progress for
sports-injury rehab, tracking dose-vs-efficacy of herbal or non-standard
treatments, etc.

In either case, I don't particularly wish to re-invent the wheel, but I'd like
to have a wheel and understand it enough to personalize it.

The fitness group is also ok with on-line and shared solutions. But the
health-problems group is concerned about privacy of data, so local control is
preferred.
