Actually, I tested with MSWord97, MSWord2002, and MSWord2007[1], and *none* of them work to properly fill out the "form". *None* of the check boxes work, nor do *any* of the form fields. On the other hand, if I use OpenOffice.org[2], *all* of the checkboxes work, and *all* of the form fields work (albeit via the popup input field box [3]).
