Hi Mark,
Are you trying to fill out the form online? You need to save a copy
to your machine, desk top would be fine, then do fill in the form.

I am not trying to fill it online. I save it, then it is in read mode
only and some of the fields did not work, then I save it locally to stop
if from being in read mode only and then I tried several times to fill
and start over again and it keeps going into read mode.
Strange thing when such an organization entranches itself on the
MSOffice platform. Pretty sad. It should really be an pdf fillable form.
