I think the problem remains here: The only place where I find this icon "Fechar" (Close) is here on separator "Menus" and not on "Barra de Ferramentas" separator (tool bar). In the tool bar I can only find the open icon.
And when following all the steps you say, I always get the answer "Icon is already there".
