[moderated] is added for moderated messages, [users] is added for
messages that go through users list.
if original poster has not bothered to write a subject, adding [users]
and possibly - [moderated] results in only these tags present
if original poster would have been kind enough to indicate what [s]he is
writing about, it would look more like
[users][moderated] <original subject>

Errrrr... are you sure about that? I'm sure someone else posted
earlier that when the subject line had only [user][moderated]
that it was because the original poster of the message was not
subscribed to the list and so the post had to be first read by a
moderator before it is sent to the list.
