As others have mentioned, any mail with "[moderated]" in it's subject line
actually indicates that the user is not subscribed and even tho an automatic
email may have been sent to them, they are not subscribed, so when you reply
to then you should also CC: them.
[Note that this holds true for all messages that are moderated, and only some
of those have the tag "[moderated]". You can always see which messages are
moderated by looking at the "Delivered-to:" header to see if one of them has
the value "moderator".
