I've confirmed that your sample file opens in OOo 2.0 and has the footnotes 
there, but numbered as zero. Deleting and reinserting the first footnote 
makes all the subsequent numbers correct again. But even after doing that, 
and saving in OOo 2 .odt file format, opening again in OOo 2.0 has the 
numbering set back to all zero. Creating a new file from scratch in OOo 2.0 
does not have that problem.

This looks like issue 56132. Could you update that issue and comment that it 
happens only with documents created before ver 2.0 (if that really is the 
case)?
