On Linux, it is held under the user's home area. I would be very surprised if Windows were different here; it is not held with the document. As a wild guess (I don't have Windows), perhaps there is a permission problem accessing the area where this information is kept. On Linux, the file is at ~/<openofficedir>/user/registry/data/org/openoffice/Office/Common.xcu - look under section AutoCorrect. Perhaps some Windows user knows where the equivalent is. I don't know whether Tools/Options - Load/Save - General, and ensure that "Load user-specific settings with the document" has any relevance.
