If you've got a Quickstarter, it should have icons for Text Document, Spreadsheet, Presentation, Drawing, and Database, exactly the same as the drop-down from the "New" icon shown in the "Getting Started with Base" document. (The Quickstarter also has "From Template".) Click Database in the Quickstarter menu and you should see the Database Wizard pop up.

Check your Windows Start > Programs menu; there should be a folder there with a series of icons in it for starting the various components of OpenOffice.org.
