Sometimes you have to select a bit of the TOC, then right-click on the
part of the TOC that's selected. Then you'll get the context menu for
the TOC with Edit, Delete, and Update Index/Table as some of the options.
I've posted tutorials for doing TOCs on my blog, in case you'd like to
look at those, as well. It's the 1.1 instructions but it works the same way.
