Greetings:
I'm not a production company, just an individual who came across a
problem similar to this on my own.  I'm not even sure if my suggestion
is more work intensive than what you are doing now.

However, would you consider the following:

1.  Use a scanner (HP makes good scanners) which will produce a scan of
the entire document as .jpg.
2.  From that .jpg file select those elements from within Open Office or
Star Office (whichever pleases you) -- using the Insert picture command
- into the document in Italian where you wish that graphic element to
appear.

Question:

My scanner had Abbyy Fine Reader embedded into it as part of it's
software, so I'm familiar with it.  Neat program.  

Did anyone consider that Open Office can read .doc as input, then use
the Edit this file icon to modify it into an Italian document (or
whatever you are attempting) as necessary then use the Save As command
to save that same document as .sxw or choose the modified or final
version of the Italian document so that it produces an output as .pdf
from within OO or StarOffice?  

Just a thought...
