[users] Alphabetical Index not in Table of Contents

Hello

I have developed a brochure in 2.0.2.
For whatever reason, the created "Alphabetical Index" that titles the 
table in the back of the brochure does not show up in the created "Table 
of Contents" in the front of the brochure.

Could not find this listed as an issue.

Any information?
