I've used PDF documents like this with forms in them (Govt. documents). But, I don't know if I could save them after I filled it out. I think I might only have been able to print them...

I do not know how if PDFs in OpenOffice can make forms, and I don't know how Adobe Reader works to fill them out. I think you would need to answer both questions to figure out an answer.

I think it is a great idea, and would like this to work. Please keep the list informed of what you come up with.
