This is simply not possible as far as I can tell. All formatting is stripped from titles and index entries.
I find it hard to believe that it has not been filed as a bug/enhancement, but if it has, I can't find it.
I expect the only workaround is to manually add the formatting. Just be aware that any manual changes will be lost completely when the index is updated.
