Sorry, about that! These methods might work IF the form is simple. Logos
and graphics will gum things up AND fonts don't always look the same.

Better approach (although tedious) is to get Scribus (www.scribus.net)
and install it. It's open source, so no expenditures for you!
Open a new Scribus file, fiddle with the page settings, make an Image
Frame (the button is in the button bar) that fills your page.
Right click inside the Image Frame and select Get Image.
Find the pdf.
Next-to-finally, click the Text Frame button (next to the Image Frame
button) and draw text frames where you want to write. Repeat this until
the form is done.
Scribus will export the file as a pdf. Alternatively, you can use
pdfcreator to "print" to an Adobe file.
