Given I can't figure out how to do the insertion in this case and
apparently no one else knows, I guess the easiest thing to do is just
delete the TOC, insert a new page, and recreate the the TOC.
I know how to insert content before the "header text", but unfortunately, I am not able to insert something before the TOC after it has been inserted. I tried pretty hard. If only I could use "insertTextContentBefore()" with a TOC index object (it works with text tables).

<sigh>.
