     It does not appear that you have done anything wrong except for 
not entering the information in the fields of the form you created. 
The form name is suppose to have (READ ONLY) after it. That indicates 
that the form, itself, can not be edited (its background, placement 
of the fields, the order of the fields, etc.). The fields within that 
form will accept information. My advice is that you open the form and 
click inside the first field. Then enter data in that field and use 
the Tab or Enter key to advance to the next field. Continue to add 
data to the fields until you have filled in all of the fields. Use 
the Tab key to go to the next record. (The number in the bottom left 
will go from 1 of 1 to 2 of 2.) The Save icon to the right of it will 
go from active to grayed out when the number changes. Now, go back to 
the main window of the database. Click on the Tables icon at the top 
left. Then double click the table associated with the form you just 
had open. It should show that data has been entered into it.
     If this does not help, please create a simple database and send 
it directly to me at the email address listed in From above.
