It does not appear that you have done anything wrong except for not entering the information in the fields of the form you created. The form name is suppose to have (READ ONLY) after it. That indicates that the form, itself, can not be edited (its background, placement of the fields, the order of the fields, etc.).
This sounds like a fairly classic usability problem, where the label is means one thing to the developers of OO but another thing to a user.
Perhaps it would be improved if READ ONLY was changed to "Form layout cannot be changed", or better still, omitted entirely from that position and indicated elsewhere.
It's like those airline pages which ask for confirmation of a cancellation with "Really cancel booking?" and then display two buttons: OK and CANCEL
