Yes, you don't need to create index entries if you use the Outline Numbering 
feature.
Hyperlinks for TOC works ONLY for styles set in the Tools>Outline Numbering 
dialog.

No, you can use custom styles but they lust be set in the Tools>Outline 
Numbering dialog for each level.


If I add 'Heading 2 Underline' to Tools > Outline Numbering I get
duplicate index entries and still no linked TOC.
The index mark has perhaps a bad influence.


Actually what I REALLY want are bookmarks in the PDF end product. Will
a linked TOC give me that or is there a better way?
OOo tags the styles used in the Outline Numbering dialog. Don't worry, you'll 
have them.
You should remove all your index entries, then configure your outlines level 
and update the TOC.
