May be this will make a difference - I've never tried it this way,
haven't got time to change it all now, but will do the next time.  Why
\../ is there two operations so similar - what's the difference between
them?

Then may be if this is 'normal' for the Tools - Outline method, I'm
correct to use the Format - Bullets and numbering... method?

The Tools - Outline method does seem to allow the referencing with
Chapter Format - but it seems to behave differently to the Format -
Bullets and Numbering option...?

No, I must have missed something - the insert button works.  But, then
it doesn't help much.  If I define some thing called 'para', then what
should I choose in the "Format" coulomb to insert - choosing "chapters"
just gives me empty references - none of the others gives the numbering
either?
