I neglected to mention another alternative: paper size. Since your top margin 
is missing, try adding an inch or 2.2 cm to your paper length in a custom 
setting, which most printer properties setups can accommodate. Also, you may 
try using inches or points instead (72/inch). It may simply be that the driver 
has the wrong conversion factor. Please let us know what you discover.

I have checked the units as you suggest and they are correctly set in

The only way I have found to produce a document with the text in the 
correct place is to print to file then print the postscript file that 
is produced by this action.
