I think it is a Zotero question, or problem I should say. Here's why:

Your errors are telling me that the Zotero extension didn't install 
correctly/completely. I just 
popped over to the Zotero site and downloaded the extension for OO and then 
installed it. I 
shut down OO (but not the quickstarter) and then called up a new text document 
from the 
quickstarter. I was then able to insert a citation from Zotero - I just used 
the default one 
included with Zotero for their Quick Start Guide. In she went, no problems. My 
system and 
versions are similar to yours.

Try downloading the extension again. Uninstall the current version in OO and 
then shut down 
OO. Start a new text document and then try adding the extension back into OO. I 
don't know 
that you have to shutdown OO each time, but we are talking about Windows here.
