     Open the Form Controls toolbar (View > Toolbars > Form controls). Click 
the checkbox icon. Then use the cursor to draw in the checkbox in the cell or 
cells where you want it or them.
        VERY IMPORTANT: In order to use the checkbox or checkboxes, you must 
first click the Edit file icon in the Standard toolbar to make the 
spreadsheet "Read-only". (It should be just to the right of the Save icon.) 
The checkboxes will only change from unchecked to checked or vise versa for 
a "Read-only" spreadsheet in my experience.
