The default grouping covers most of my needs and indeed I have often
modified the heading and index parent styles. I found combining the
linking styles with font size expressed in percentage quite powerful
(e.g. heading 1 = 120% of heading, heading 2= 100% of heading and so
on...)
I have only added the following linked styles:
1) paragraphs with different language properties (English, Italian)
2) characters with different language properties (English, Italian, no language)
3) paragraphs with different font colour properties (for when I write
in a frame with different backgrounds)
4) character styles with different highlight colours
I also created a numbering paragraph style with an associated list
style containing below numbering start, numbering continue, numbering
end, but I never used it at the end (much quicker to use list styles).

Again, this is not a general advice but just my experience: I have
heavily modified the predefined styles and then saved them in a
template that I now use instead of the default template. I also added
some more styles that I needed in the documents I write.
