I think you have to delete the index and clear the fields in the text
separately.
To delete the index itself, right-click in the index itself and
choose Delete Index/Table from the context menu.
To clear the fields where the entries originated:
o Find the first such field in the document and put the cursor in it
(or immediately before it).
o Go to Edit | Index Entry... or right-click | Index Entry... .
o In the Edit Index Entry dialogue, select Delete repeatedly as the
fields are individually deleted and the focus moves through all of them.
