In addition to other help you've been offered let me suggest an additional step that can help to protect the information (PHI - private healthcare information per HIPAA) contained in this file. If you'll be the one person using it encrypt it with a password. That way no one else can open it as long as you don't share the password. And if no one can open it the information can't be accidentally divulged and no HIPAA violation can occur from loss of the file. Presuming you will use Calc and the Open Document Format (.ods) as your file type... If you've already created the file, do the following: 1. open the file 2. select File > Save As... from the menu 3. check (tick) the "Save with password" option near the bottom of the window 3.a. make sure the "Automatic file name extension" option is checked (ticked) as well 4. click "Save" 5. A message will display asking whether to replace the file, click "Yes" 7. enter a password (it must be at least five characters long) 8. enter the password again 9. click "OK" You've now saved the file with a password. This encrypts the file. DO NOT LOOSE or FORGET the password. The file cannot be opened without it.
