On the occasion that I really need a booklet, I do it the way I do any
booklet in Writer: I set up a custom page size for a single page in the
final booklet (i.e. 12x12cm here), create the booklet as a normal
document--text flow is all normal, print to Postscript, and use a little
script to re-arrange the pages for booklet printing.

But as I noted, the printer (in fact, both of the ones I have)  loses
enough of the page edge to be a problem with a small page.  I already
use psbook for some purposes - but it doesn't help the margins :-(

I think I need a way to lay it out on larger paper to print before
cutting to size. But how..... I've even been looking at scribus, but
afaict the page ordering still has the exact same issue :-{

I know that Microsoft is not something we like, BUT some of the
DOC templates work well.  They had CD covers and such.
Also AVERY allows you to download their templates for free as well.

Could there be a template already made that will work for you?
I do not use A4 paper size.  I use 8.5 by 11 inches paper.  I find that
if I use it in landscape I can get two CD insert lables per page.
I use OOo for most of these things.  I have been making DVDs
out of video clips for Trains and Railroad related things for a person
who is disabled and only has TV/DVD on his computer.  I find that
the landscape double sided printing works fine for me.  That gives
4 "faces" of the lable insert in the standard CD slim case.  Though I
have a template for the original size CD case that uses both the opening
flap lable and the one that goes in the back of the case.

Let me know exactly what you need --- off the list --- and I
will see if I can help you find want you need.  I use a Graphics
program (several in fact) to make the images and graphics, and
then I insert it into a Writer page.  Works for me.  If I cannot
get Writer, etc., to work, then I fall back on Adobe PageMaker.
Almost always I use Writer, and it works well.
