It appears that it's an OOo rtf conversion/filter problem. I tried the
rtf in OOo 2.3.0 (Ubuntu/Novell), OOo 2.4rc1 linux, OOo 2.3.1 Windows,
Abiword linux, and MS Word 2000. Only the MSW2000 brought it up
properly. Abiword did a best effort; meaning that it didn't show the
graphic but got the frames correct, but not much else. However, when I
saved the rtf from MSW2000 to a .doc file and then opened in OOo the
file comes up fine, including all check boxes. Hence my guess that it's
a OOo rtf filter problem.

I'd suggest that you file a bug report using the same document and the
PDF's as attachments to the report. Hopefully the developers can figure
out the problem & correct before OOo 3.0 (I think it's probably too late
for 2.4).
