 Attached is a #10 envelope, I do not remember where I got it, but it
 works pretty well.

 Generally you insert into your document the envelope with INSERT =>
 ENVELOPE.  I suggest that you print first on plain paper to see how the
 formatting works, before you waste envelops until you have the placement
 set up.
